We have made our New Patient forms available on our website for your convenience. Please click on CHILD or ADULT to print out your form and fill it in prior to your first appointment. If you have insurance coverage, please bring your insurance card, form or benefit pamphlet with you. Also, please have your section of the form filled in with all pertinent information before your appointment so we can see you promptly.
It is our desire to provide for your orthodontic needs both thoroughly and efficiently. The initial appointment includes your meeting with the doctor, reviewing a clinical diagnosis, discussing a treatment plan, reviewing all payment options and, when indicated, taking impressions of the teeth.
